Our Street Outreach program is led by our case managers, who go directly into the community, encampments, sidewalks, and parks, to connect with unhoused individuals. As part of our CalAIM program, they provide on-the-spot support, helping people access housing, healthcare, benefits, and other vital services. By meeting people where they are, we build trust, offer consistent care, and help folks take the next steps toward stability.
